Tournament Report Form

Before you begin to fill out your Tournament Report, be sure you have everything you need to complete the submission.

FIRST – Download the Excel Sheet and Fill out your Officials information.

SECOND – Be sure to have your USFA Classification Change Report form ready, IF you have changes to report. If you have no changes to report, you will not need to include this form. Please Click Here to Download the USFA Classification Change Report Excel Sheet

Once you have your forms complete you are ready to fill out your Tournament Report. Note: Your Tournament Report Form will not be complete or accepted without uploading a complete Officials Excel Sheet and an updated USFA Classification Change Report.

BEFORE you submit your online form, be sure to print your Tournament Report and mail it with payment to:

Starting Oct. 15, 2015 the USFA is requiring tournament report / classification change forms to include the names of the referees. All referees over the age of 18, now have to have the Safe Sport background check done or no classification changes will be approved and fines could be levied.
